* Please note times listed for physical therapy and performance medicine sessions are approximate. Sessions may run slightly shorter or longer, as long as our goals for that session are achieved.

Insurance
For clients with Medicare, please click here!
JMPT does not accept insurance for physical therapy services and is an out-of-network provider. Most clients have a PPO plan that allows them to use out-of-network benefits, and many clients are covered for the majority of their treatment costs by their insurance provider.
A detailed invoice — also known as a Superbill — will be provided to you via e-mail, which you can file with your insurance provider for reimbursement.

cancEllation/no-show policy
If you must cancel an appointment, please do so no later than 24 hours prior to your scheduled appointment time. If you are unable to give a 24-hour notice, or if you no show for your appointment, there will be a cancellation fee equal to 50% of the cost for that session.
