Musculoskeletal rehabilitation
Joint or spinal pain, movement limitations, or strength deficits affecting daily activity, exercise, or work.
Postoperative recovery
Rehabilitation after orthopedic procedures, including ACL reconstruction, meniscus repair, rotator cuff repair, or joint replacement, guided by surgical precautions and protocols.
Sports & return to activity
Patients working toward running, lifting, recreational sport, or other physically demanding goals through progressive rehabilitation.

Focused care, built around the patient
Patients work directly with Joe throughout their care, with rehabilitation connected to their daily activities and goals.
Initial evaluation & treatment
A 90-minute initial evaluation provides time to assess symptoms, movement, strength, and functional limitations and establish an individualized plan.
One-on-one follow-ups
Standard in-person follow-ups are 60 minutes. Care may include progressive exercise, movement retraining, hands-on treatment, and home programming.
Progress toward meaningful goals
The plan focuses on the activities the patient wants to regain, with exercise and activity progression guided by their response and reassessment.
Postoperative considerations
Include the procedure, surgery date, precautions, and surgeon’s protocol with the referral. Discuss any restrictions or questions that need clarification before treatment.
Visits may take place at home, an office, a gym, or an appropriate outdoor setting. To safely bring in the treatment table and equipment, the patient's location must be accessible, with relatively easy parking and reasonably easy access to the treatment area.
Care coordination & communication
Connect directly with the treating physical therapist to align the referral with your patient’s needs and your clinical plan.
Share the clinical context
Include the reason for referral, relevant findings, precautions, and postoperative instructions, along with your office’s contact information.
Agree on follow-up
Discuss your preferences for an evaluation summary, progress updates, discharge information, and communication about concerns.
Clarify the handoff
Confirm who will contact the patient and how your office would like to be informed about scheduling or barriers to starting care.
Coordinate clinical information through an agreed channel, with the patient’s authorization as appropriate.

Insurance & payment
Commercial insurance & self-pay
JMPT is out of network with commercial insurances. A detailed invoice (superbill) may be provided for patients to submit to their insurance provider for possible reimbursement.
Benefits and reimbursement depend on the patient’s plan and are not guaranteed.
JMPT accepts FSA/HSA cards for physical therapy services, as well.
Medicare
JMPT accepts Medicare and is enrolled to bill Medicare only. If another insurer is primary, confirm the patient’s arrangements with Joe before scheduling.
For patients with Medicare, initial evaluations and follow-up sessions are 60 minutes.
Please note availability for Medicare may be limited. For post-surgical cases Joe will do his best to schedule your patient as soon as possible.
Provider questions
Does the patient need a referral or order?
Contact Joe to confirm what is needed for the specific patient. Access to physical therapy and payer documentation requirements are separate considerations; requirements may depend on the patient’s circumstances and coverage.
How do I send postoperative protocols?
Fax the protocol and relevant precautions to (415) 276-4783, with the patient’s name, procedure, surgery date, and your office contact information. Call to clarify any time-sensitive restrictions.
Is mobile physical therapy the right service?
The practice focuses on orthopedic and sports rehabilitation at the patient’s location. If the patient needs a home-health episode, nursing support, or a different level of assistance, discuss those needs with Joe before referring; a visit at home alone does not establish eligibility for a particular service or benefit.
What if the patient’s age, needs, or location may be outside the practice’s scope?
Call Joe before arranging the referral to discuss suitability, the visit address, and availability. Confirm the appropriate service before sending the patient for care.
